精神病性障礙個案⾃殺預防之護理經驗:以⽣成式⼈⼯智慧作為輔助⼯具之個案報告
Nursing Experience in Suicide Prevention for a Patient with Psychotic Disorder: A Case Report Using Generative AI as A Supportive Tool
【摘要】
本個案報告描述⼀位 30 歲男性因長期使⽤安非他命後出現幻聽、⾃傷及⾃殺風險之照護經驗。個案最初診斷為物質相關精神病,惟於戒斷後症狀仍持續,後續評估為「非物質或⽣理因素所致之精神病性障礙」。本案護理期間為 2025 年 1 ⽉ 8 ⽇至 2 ⽉ 7 ⽇。護理過程透過直接觀察、治療性會談及病歷查閱蒐集資料,並依⽣理、⼼理、社會、認知與靈性五⼤層⾯進⾏整體性評估,確⽴主要護理健康問題為「潛在危險性⾃殺⾏為」及「⼼靈困擾」。照護重點在於建⽴護病關係,並結合正念認知療法、⽣命回顧,以及以⽣成式⼈⼯智慧作為輔助⼯具協助製作⽣命故事書,透過敘事歷程陪伴個案整合⽣命經驗、重整⽣活⽬標並強化家庭連結。另以簡式健康量表(Brief Symptom Rating Scale,BSRS-5)進⾏每⽇情緒與⾃殺風險監測,並配合病房安檢、團體治療及藥物治療。住院期間,個案未再出現⾃傷或⾃殺⾏為,出院前亦能表達對重建家庭關係、遠離物質使⽤及回歸社會之正向期待。

Abstract:
This case report describes the nursing experience of a 30-year-old man who developed auditory hallucinations, self-injurious behavior, and suicidal risk following prolonged methamphetamine use. He was initially diagnosed with substance-induced psychotic disorder. However, psychotic symptoms persisted after detoxification and were subsequently determined to be consistent with a psychotic disorder not attributable to substance use or other physiological conditions. The nursing care period extended from January 8 to February 7, 2025. Data were collected through direct observation, therapeutic interviews, and medical record review. A holistic nursing assessment encompassing physiological, psychological, social, cognitive, and spiritual dimensions identified two primary nursing problems: risk for suicidal behavior and spiritual distress. Nursing interventions focused on establishing a therapeutic nurse–patient relationship, implementing mindfulness-based cognitive therapy (MBCT), facilitating life review, and applying generative artificial intelligence (AI) as a supportive tool in the development of a personalized life storybook. Through this narrative-based process, the patient was supported in integrating life experiences, reflecting on personal meaning, and strengthening family connections. Daily emotional status and suicide risk were monitored using the Brief Symptom Rating Scale (BSRS-5), in conjunction with routine ward safety checks, group therapy, and pharmacological management. During hospitalization, no further self-harm or suicidal behaviors were observed. Prior to discharge, the patient expressed positive expectations regarding rebuilding family relationships, abstinence from substance use, and social reintegration.
